Health Care Referrals – North Simcoe Muskoka

  • Poverty affects health through many influences including material deprivation and chronic stress, contributing to the development of medical conditions
  • People who visit a primary care provider for a medical issue, may be eligible for social services or other supports they may not be aware of
  • While primary care providers are increasingly aware of the impacts on health due to social determinants, they may not be aware of programs and resources that are available to address social issues and may not have the time themselves to discuss these resources with patients or connect them directly
  • Partnerships with the Family Health Teams in Collingwood and Orillia enhance their clinician’s use of a clinical tool which helps screen for poverty concerns in patient encounters, through an e-referral to 211
  • 211’s Community Navigators receive e-referrals, contact patients to provide mediated assessments, advocacy and follow up to connect them with needed services or programs
  • Outcome reports on the interventions provided by 211 and results of connections to services are collected, and summary reports provided to the referring clinician for each patient
  • An e-referral to 211 is used as a mechanism to bridge the gap between primary care and community/social services/government supports for patients identified as living in poverty )or at risk) and impacted by the social determinants of health.

Child Youth and Family Services Coalition of Simcoe County Mini Directories

  • The Coalition is an alliance of organizations providing services to children, youth and their families in Simcoe County.
  • Community Connection’s Resource Specialists developed and maintain a set of three custom online directories for youth, francophone youth and First Nations, Métis & Inuit accessible through the Coalition’s website http://www.simcoecountycoalition.ca/need-help/.

Children’s Treatment Network of Simcoe-York

  • The Network provides comprehensive care and services to kids with complex physical, developmental and communications needs through a coordinated Single Plan of Care.
  • 211’s Community Navigators provide call centre overflow support to the Network’s Navigation Team ensuring callers have an opportunity to speak with a Navigator with their first call.

 Community Directories for Simcoe and Muskoka

  • The Simcoe Muskoka Workforce Development Board is a community directed, non-profit corporation leading Simcoe County and the District of Muskoka in their approach to workforce development and labour market planning.
  • Community Connection’s Resource Specialists provided the community resources used to develop two community directories:

Muskoka Community Directory.

Energy Audits – Bruce and Grey Counties

  • In partnership with the United Way and Union Gas, qualifying customers can receive a complimentary energy audit to identify free energy upgrades (e.g. showerhead, kitchen and/or bathroom aerator, programmable thermostat, draft proofing, attic, wall or basement insulation) to their home to help reduce their monthly energy bills.
  • Community Connection’s Community Navigators provide centralized intake for residents who may quality for this home weatherization program and provide additional referrals to other community supports.

 Food in Simcoe County

  • Community Connection’s Resource Specialists partnered with the Simcoe County Nutrition Network and the Food Partner’s Alliance of Simcoe County to design to develop online access to information about affordable food access and security.
  • Community Connection’s Community Navigators use this resource to provide comprehensive information and referrals to callers seeking food resources www.foodinsimcoecounty.ca.

 Free and Low-cost Daily Meal Calendars

  • Community Connection’s Resource Specialists maintain online calendars of the daily meal programs available across the Central East Region. 
  • Community Connection’s Community Navigators use this resource to provide detailed information about food access to callers in need.

 Municipal Protocols for Emergency Response

  • In partnership with the Office of the Fire Marshal and Emergency Management, Field Officers and Community Emergency Management Coordinators to develop municipal communications protocols with 211.
  • Community Connection has confirmed protocols or agreements with 29 upper and lower tier municipalities.

 Railway Suicide Prevention – Northumberland County

  • To offset the many calls for assistance along Northumberland rail lines to local police services and rail police services, Northumberland Safe Communities is posting signs they hope will have a high impact on individuals who will stop to read the sign and call 911 or 211 to reach out for help. 
  • Northumberland County does not have a 24/7 crisis line and the partnership with 211 will ensure callers will get connected to the professional services they need anywhere in Ontario, 24/7 in 150+ languages.

Simcoe County Community Paramedicine Program

  • Community paramedicine allows paramedics and emergency medical technicians (EMTs) to operate in expanded roles to provide routine healthcare services to underserved populations, and helps to improve rural emergency medical services (EMS).
  • 211’s Community Navigators receive referrals from Community Paramedicine and contact each patient to provide a further assessment, ensure they understand the information and referrals provided.   

Simcoe Muskoka Vulnerable Populations Emergency Plan

  • The vulnerable populations plan identifies strategies and provides communication mechanisms to support identified needs of vulnerable populations in Simcoe County and the District of Muskoka in the event of an incident or emergency.
  • Community Connection supports the communications plan as a level 2 agency with 24/7 capabilities to distribute notifications/communications to assigned agencies who serve vulnerable residents.

 Utility Assistance – Bruce and Grey Counties

  • Winter Warmth is emergency financial assistance, coordinated and delivered to customers by United Way of Bruce Grey through a network of community agencies within Union Gas’ service area.
  • Community Connection’s Community Navigators provide centralized intake for financial assistance and provide additional referrals to other community supports.
